8/5/2023 EnchantaVaughn Approved Behavior: QMHPA greeted Ms. Broadnax upon arrival to the home setting. Upon arrival to the home setting, Ms. Broadnax appeared to be in a good mood evident by her facial expressions such as a smile on the face. QMHPA inquired about her afternoon. Ms. Broadnax verbalized to QMHPA that her afternoon was going good. QMHPA informed Ms. Broadnax that this session was going to continue to focus on managing her depressive symptoms. Upon arrival, she did not present any symptoms of S/I. Intervention: QMHPA began the session by inquiring about Ms. Broadnax’s current thoughts, feelings, and emotions. QMHPA engaged Ms. Broadnax in a discussion surrounding her medication management. QMHPA discussed with Ms. Broadnax the importance of scheduling important medical appointments and attending them and not cancelling them on a regular basis. QMHPA discussed with Ms. Broadnax she is to attend all schedule appointments without any excuses. QMHPA engaged Ms. Broadnax in a skills building activity. QMHPA informed Ms. Broadnax that the purpose of this activity was for QMHPA to guide Ms. Broadnax through a series of questions where she will identify signs of depression and situations that causes her to become depressed. QMHPA questioned Ms. Broadnax about different techniques she applied to overcome depression and recommended she apply those methods now. QMHPA questioned Ms. Broadnax about how her depressive disorders affect her and those around her. QMHPA assisted Ms. Broadnax with identifying her mood swings and alternative methods of dealing with her mood swings when they occur. QMHPA encouraged Ms. Broadnax to display her emotions in a positive behavior avoid becoming frustrated, lashing out and throwing objects. Response: Ms. Broadnax appeared to be receptive to QMHPAs’ interventions and services that were provided to her during the session. Ms. Broadnax said she will become quiet when depressed, cry, frustrated at things around her, and her blood pressure may rise. Ms. Broadnax indicated to QMHPA that she just seems depressed on a daily basis and something has to come to an end. Ms. Broadnax admitted to having occurring mood swings and is able to recognize her behaviors. Ms. Broadnax said she would isolate herself and just become angry with everyone. Ms. Broadnax said she can’t control her behaviors all the time she said she will be good at one moment talking and joking around then she will just become angry.
